SALES CONTRACT & HEALTH GUARANTEE FOR PET QUALITY COMPANION
Definitions used herein include:
“Cockapoo” shall herein be defined as a Cocker Spaniel and Poodle Hybrid mix dog
"BUYER" shall herein be defined as:
"BREEDER" shall herein be defined as: Kristin Coppom of Sandy, Oregon
“PUPPY” shall herein be described as a Cockapoo

The Cockapoo described above is sold for the sum of $ .
A pet puppy is defined here to be a Cockapoo puppy which is in good health at the time of sale, and embodies the characteristics of the Cocker Spaniel & Poodle breeds. This PUPPY is not registrable. It is understood that at the time of sale this dog is not considered to be show quality, but is representative of the respective breeds and is structurally and temperamentally suited as a companion dog. The BREEDER guarantees the above described dog to be healthy at the time of the sale. The BREEDER does not assume any liability for any injury to said puppy after delivery. The BUYER agrees to have this puppy examined by a veterinarian within 72 hours of delivery. If the puppy is found to be other than healthy by the BUYER's veterinarian, it shall be returned to the BREEDER immediately, and the purchase price will be refunded.

HEALTH GUARANTEE
PUPPY is guaranteed against congenital or hereditary defects for a period of one year from the puppy's date of birth. In order to arrange a replacement of the puppy, you must obtain a written diagnosis of a non-curable congenital problem from a licensed veterinarian. THE FOLLOWING ARE NOT TO BE CONSIDERED HEALTH PROBLEMS: Worms, ear-mites, fleas, diarrhea caused by feeding non-recommended foods, and/or cows milk(unless in current diet), injuries occurring after the sale. Also, hypoglycemia (Low- Blood Sugar and Pattellar Dislocation, or luxating Patella (slipping kneecap) are not health problems. Breeder will provide minor treatment for some of these listed exclusions either by phone, email or in person. Any veterinary treatments for the excluded health concerns are the sole responsibility on the buyer and in no instance shall Breeder be liable of such expenses. All vet bills are the responsibility of the buyer. All appropriate shots for the PUPPY'S age have been given, and a record of this
vaccination history is supplied.
The BUYER agrees to maintain the puppy's health in good condition, and to provide yearly examinations, vaccinations, and any other usual procedures necessary to assure good health. The BREEDER has done everything possible to produce healthy sound puppies by raising the puppies in the best way possible in regards to diet, exercise, socialization and veterinary care. There are many factors that can influence growth and development, such as diet, exercise, weight gain, etc.. The BUYER assumes the responsibility to carry on with the appropriate vaccinations, exercise and feeding schedule, as supplied by the BREEDER. The BREEDER can offer no further guarantees once the care of the puppy is no longer under their control. The BUYER hereby agrees that if at any time the Buyer cannot take proper care of, or keep the puppy, the BUYER may contact the BREEDER and the BREEDER will have the first choice to either: (a) take the PUPPY back or (b) assist the BUYER in finding a suitable
home for the PUPPY.
The BUYER agrees to provide a suitable fenced area for this puppy.
The BREEDER neither makes or implies any warranties or guarantees, express or implied, other than those written in the agreement including, without limitation, the warranties of merchantability and fitness. This document constitutes the entire agreement between the Breeder and Buyer with respect to this sale. The Buyer's signature below indicates that he/she has read, agrees and does understands all the conditions of the Sales Agreement and Contract of Sale.
